Passport name change

If you need to update your passport due to a name change (marriage, divorce, or court order), the process and cost depend on how recently your current passport was issued. There are two pathways: Form DS-5504 (free correction within 1 year) or the standard renewal process using Form DS-82.

5. How to Submit

  • Mail Form DS-5504 with documents to the address listed on the form
  • Use a trackable delivery method
  • Your new passport will be mailed to you; old passport returned separately

  • Issued when you were 16 or older
  • Issued within last 15 years

7. Cost

  • $130 for passport book renewal (standard renewal fee)
